How to access your records
Under the Data Protection Act 2018, you have a legal right to apply for access to health information or other personal data held about you. In addition, the Access to Health Records Act 1990 gives the personal representative of a deceased patient or someone who has a claim arising from their death, a right to request access to the deceased patient’s health record.
If you’d like to access health records or personal data held by St Oswald’s Hospice, we have a form which can be returned via email to datasubjectrequest@stoswaldsuk.org or alternatively you can email describing the information you require.
The timescales for responding to your request will be dependent on whether this is a Data Protection request (for yours or another living person’s personal data) which are usually responded to within a month or an Access to Health Records Act request, where we will respond within 21 days when the record has been added to within the previous 40 days, or in other cases, within 40 days.
